Extracting the kind of information you want from students in these evaluations is not always so easy, but here are some reasonable options:

  • Questionaire 1 - For problem-solving sections. Uses 1 to 4 scale for seven problems, with room for comments. Well suited for generic math courses.

  • Questionaire 2 - For problem-solving sections. Similar to the first one, with slight variations in the questions and tone.

  • Questionaire 3 - For discussion sections. It is similar to the previous two, again with slight variations in the questions and tone.

  • Questionaire 4 - For problem-solving sections. While it uses a 1-4 scale, this form essentially begs students to give more complete answers. Very nice.
You may also wish to design your own form. Former Head TAs Jason Lee and Cameron Parker created an alternate evaluation form which has been used widely by math TAs. Here is an adapted version of that form as a pdf and as a TeX file (for easy editing).
